HARDWICK CLINICS TERMS & CONDITIONS

CONSULTATION AND PRICES
At Hardwick clinic every new patient is offered a “free aesthetic consultation” which can be booked via our online booking system. All our patients receive comprehensive treatment and care, provided by highly trained and experienced practitioners. We carefully select our products which are fully licensed in the UK and supplied by British firms. Our prices are available online and are confirmed prior to a procedure during the consenting process. We are unable to offer refunds for our procedures. We offer one “free top up” treatment within the 14 to 21 day period following botulin toxin treatment. We are also able to offer complimentary follow up consultations, and “10 minute follow-up” appointments to address any questions after any treatment, bookable through our booking system. We currently ask for a deposit to book a treatment consultation with us.

CANCELLATION AND LATE ARRIVALS
Cancellation can be made by text, email, phone or via our online booking system. If you can’t make your appointment, please let us know as soon as possible. If you miss your appointment or cancel less then 24 hrs notice  your appointment booking deposit is non refundable or for  treatments that are part of a course  the treatment becomes forfeited.  You will receive 24 hrs alert notification prior to your appointment. We reserve the right to refuse future appointments for repeated non-attenders. We do our best to accommodate late arrivals, however, there may be times when a late arrival may result in reduced consultation time, or we may have to reschedule your appointment.

INSURANCES AND REGISTRATIONS
Hardwick Clinic is fully registered with Save Face and JCCP which are both regulatory bodies in the Aesthetic industry. Hardwick Clinic is fully insured clinic including public liability insurance. Hardwick Medical Clinic is a subsidiary of Hardwick clinic with CQC registration. All of our practitioners hold relevant professional registration and full professional indemnity insurances.

COMPLAINTS
Should you have any concerns about your treatment or would like to make a complaint, please contact the clinic directly by telephone or email. Should this not be sufficient you can complain to the Independent Doctors Federation (IDF) directly.

We understand the importance of ensuring that the personal information entrusted to us is protected and respected and we therefore treat personal data in accordance with our obligations under the General Data Protection Regulation, in addition to our clinical confidential requirements.

Hardwick Clinic is registered in England with the company number 08800507 for provision of cosmetic services, VAT registration number is 427509292. Hardwick Medical is registered in England with the company number 08802608 for provision of medical services.

Medical Records – Information must be collected as part of your registration with the clinic. This includes information such as your name, address, date of birth, e-mail address, phone number, next of kin and medical history.

Failing to provide us with this information will result in us being unable to provide treatment.

Enquiries – We may record your details in order to track and follow-up on enquiries, in addition to scheduling appointments. This includes information such as your name, e-mail address or phone number.

Failing to provide us with this information for marketing purposes will have no impact upon the provision of your treatment.

Information we collect automatically

With regards to each of your visits to our website drmedispa.com we will automatically collect the following information:

Technical information, including the Internet Protocol (IP) address used to connect your computer to the internet, your login information, browser type and version, time zone setting, browser plug-in types and version, operating system and platform; and

This includes the police for the prevention or investigation of a crime or our Insurers, legal advisors or other third parties who need access to it in the context of managing, investigating or defending claims or complaints.

YOUR INFORMATION
We cannot guarantee that the information you send us over the internet is secure, but once we receive it we will take all reasonable steps to protect the information you supply to us. We will not transfer your data outside of the European Economic Area. Backups of electronically stored data are taken regularly, with strong encryption used to protect all files.

Under the General Data Protection Regulation, we will only hold personal identifiable data for the maximum retention periods:

Job applications and unsuccessful interview candidates – 6 months
Employee Records – 6 years following termination of contract
Medical (Aesthetic) Records – 7 years following last appointment
Medical (GP & Hormone) Records – Indefinitely
Controlled Drugs Register or Prescription Information – 2 years
Marketing Enquiries and Emails – 6 months
Financial Records – 7 years

ACCESS TO INFORMATION
